There are several ways to begin services. Call 304.525.7851 for an appointment or walk-in to a location near you any weekday (Monday through Friday) during regular business hours (8:00 am to 5:00 pm)

What to Bring to Your First Appointment:
- Identification
- Proof of Residence
- Proof of Income
- Social Security number and date of birth
- If you have insurance, bring the insurance cards and the following information for the insurance subscriber: Social Security number, date of birth and name of employer.

- WV Medicaid and WV Medicaid Managed Care
- Medicare
- Self-Pay
- Commercial Insurance
- Note: We do not accept Kentucky or Ohio Medicaid
Employee Assistance Programs (EAP):
- Employees of businesses with established EAP agreements can call 304.525.7851, x1221 to schedule an appointment

Video Teleconferencing
Services may be provided through a secure video conferencing connection (sometimes called “tele-health”). Video teleconferencing minimizes barriers to accessing services.
