PATH

OVERVIEW

Prestera Center’s mission is to promote recovery and self-actualization by empowering individuals and offering services that support people in finding the hope and dignity needed to live more meaningful and enriched lives. The PATH program is housed at Harmony House located at 627 4th Avenue, Huntington WV. The Huntington program’s five main goals include providing care for immediate needs, ensure safety when indicated, provide crisis intervention, and refer to immediate medical care (including mental health), help consumers with immediate clothes, food, and shelter needs, and link to homeless and mainstream services. The purpose of the program will be primarily to engage homeless people who are (1) underserved for a variety of reasons, such as having a lack of trust in agencies, (2) underserved people have a multiple of needs but for many and varied reasons are not receiving all the help they need, (3) people who have been rejected from a variety of services and programs due to drug or alcohol problems or other anti-social or non-compliant behaviors. All the above people further fit the definition of chronic homeless and will have serious mental illness and co-occurring substance abuse issues. These people are the most difficult to reach and in need in our community.

QUALITATIVE REVIEW

  • Fidelity  Program will fall under the ideology of PRESTERA CENTER. The program will run under ethical services and supports that are in the interest of the individuals seeking or agreeing to assistance. The supports will follow under the “promising practice”, a model for which there is considerable evidence or expert consensus but are not yet supported by “rigorous” scientific evidence.
  • Stakeholder Involvement / Direction   Will encourage and seek out support from individuals that seek out intended services. Will request direction and allow individuals the opportunity to make recommendations to improve the distribution of supports/services. The diverse stakeholders (include but are not limited to: individuals in the program, members of their families, staff directly providing services, community representatives) participate directly in, and provide input into decisions about: 1) program development and planning, 2) implementation, 3) evaluation and 4) modification.
  • Efficacy  Utilize the five main established goals to meet intended results. The goals and objectives specifically identify significant, positive changes that will occur in the lives of the individual consumers as a direct result of the program’s activities, or consumers report progress in their respective recovery process as a direct result of the program’s activities.
  • Accessibility / Public Awareness  Provide services at Harmony House to provide the most significant outreach possible in the area. Increase public awareness through meetings with various organizations, media, etc. The program’s activities include a specific marketing/public awareness plan that utilizes some form of media targeted to people directly involved in the program. Potential consumers, members of their families and other stakeholders have at leave one opportunity to learn about the details of the program.
  • Environmental Integration  In accordance with state and local law provide access to all individuals regardless of physical and developmental limitations. The program will provide supports to all individuals in need regardless of race, sex, sexual orientation, religion, age, or any other areas that can discriminate. The program operates in a community setting with no apparent distinctions among consumers, staff and other members of the community. The concept of community integration is integral to all facets of the program and is concretely highlighted in the program’s written materials and in the conversations that occur in its implementation and reporting activities.
  • Recovery Promotion  Will provide individual’s supports that can assist them in meeting and obtaining support goals. Including but not limited to housing, mental health services, medical services, etc. The concepts of recovery and resilience are integral to all facets of the program and are concretely highlighted in the program’s written materials and in the conversations that occur in its implementation and reporting activities.
  • Staff Knowledge and Skill  Staff will demonstrate skills through experience and agency training. Continued skill development will occur through technical support provided by West Virginia’s Mental Health System. A written job description, including knowledge and skills for staff. The staff exhibit skills evident in verbal evidence and have the requisite knowledge and skills.
  • Cultural Competency  Staff will utilize their own cultural experience to direct them in supporting others and be willing to embrace others culture in order to assist the individual get out of their situation of homelessness. The program has a mission statement and policies that discuss cultural competency and has a good understanding of the different cultures it serves but the program does not address cultural competency in the services provided or plan of delivery.

70% of the consumers will report satisfaction with the PATH program.

30% of the individuals referred will obtain stable housing.

50% of the people will be able to remain out of the hospital while in the PATH program through support referrals.

85% of the consumers will obtain immediate clothing, food, and shelter as requested.
The Continuum has identified approximately 50 people who will be engaged throughout the year.
