Description of Program

The Medicaid Waiver program serves individuals who have a primary diagnosis of MR/DD and/or developmental delays. Dually diagnosed individuals are also served if approved for waiver services by the WV Office of Behavioral Health Services. Services include: service coordination, respite care, agency residential habilitation services, community residential habilitation services, adult companion services, QMRP services, nursing services, day habilitation services, pre-vocational services, supported employment services, and transportation services.

Days/Hours of Operation

Services are provided in the Consumers home and in the community. Services are offered daily up to 24-hours per day.

Admission Criteria

  • Diagnosis of MR/DD or Related Condition as outlined in the Waiver Title XIX manual.
  • Approval for Waiver Title XIX services by the WV Office of Behavioral Health Services.
  • Completion of required DD Assessment and reports by provider
  • Involvement in some form of day habilitation programming (day treatment, community day hab, etc.)
