Boone County Day Habilitation

PROGRAM LOCATION / ADDRESS

376 Kenmore Drive
Danville, West Virginia

DAYS/HOURS OF OPERATION

Monday through Friday: 8:30 a.m. to 2:30 p.m. or up to six hours per day.

STAFFING PATTERN

The Program will be staffed with 5 FTE BRS and a one-half time Therapeutic Consultant. Supervision will be provided by the Boone County Supervisor, Area Waiver Supervisor and a MA level program director. The program will be staffed a 1:3 ratio.

ADMISSION CRITERIA

  • Diagnosis of MR/DD or related condition as outlined in the Title XIX MR/DD Waiver Manual.
  • Approval for Title XIX Waiver Services by the West Virginia Office of Behavioral Health Services.
  • Completion of required DD Assessment Waiver Cost Estimate to include Day Habilitation and the need of Day Habilitation specified on the DD-5.

CONTINUING STAY CRITERIA

  • The service is necessary and appropriate to meet the consumers’ needs as identified on the Individual Service Plan.
  • Continued approval and certification for Medicaid Waiver Services by the Office of Behavioral Health Services.
  • Completion of required DD assessments and reports by the service coordination agency.

DISCHARGE CRITERIA

  • Consumer’s family and/or guardian request discharge or transfer to another provider.
  • Consumer masters skills necessary for independent living.
  • Death of the Consumer.
  • Consumer no longer meets eligibility requirements for the MR/DD Medicaid Waiver Program.
  • Physician orders dictate consumer’s inability to participate in Day Habilitation.

DESCRIPTION OF PROGRAM/SERVICES/POPULATION SERVED

The MR/DD Title XIX Waiver Program serves individuals who have a primary diagnosis of MR/DD and/or developmental delays. Dually diagnosed individuals are also served if approved for waiver services by the West Virginia Office of Behavioral Health Services. Services include Therapeutic Consultant, Supported Employment, Transportation, Adult Companion and Day Habilitation Services. The Program will provide training in new skill areas in order to improve the quality of life and to enhance independence. The population served will be 18 years or older.

PURPOSE

The Day Habilitation Program provides individuals who require an ICF/MR level of care and who are otherwise eligible to participate, training opportunities to attain independence, personal growth and community inclusion and to improve their quality of life. Said opportunities are offered in a safe, clean, and therapeutic environment.
