Description of Program

The In-Home Services program case manager provides targeted case management, such as advocacy, linkage, referrals, assessments, and service plan evaluations. The case manager also provides supportive intervention services sessions to improve day-to-day coping and problem-solving skills if identified on the treatment plan as a need. The in-home services therapist primary responsibility is to provide individual therapy and family therapy to the consumer. The IHC team conducts on-going assessments of the consumer’s individual strengths and needs, schedules and facilitates treatment team meetings, links consumers with needed services and communicates with individuals relevant to the consumer’s treatment, and serves as an advocate for the child and their family when necessary. The IHC team ensures that the services respect the needs of the child and the family according to their culture (ie. urban vs. rural). In the initial phase of treatment the team will provide at least two hours of face-to-face contact per week per client. Each consumer and family will participate in treatment planning and assessment of current functioning to ensure all areas of need are addressed. These activities are conducted primarily in the consumer’s home but can be performed at the primary office location of the team.

Mission Statement

The goal of the In-Home Services Program is to preserve the child within the family unit. The IHC teams will provide quality case management & therapy services for children with serious emotional disorders. These services will be strength-based, individualized and culturally competent. To achieve this mission the IHC teams will work collaboratively with other agencies, participate in on-going training to increase their knowledge of issues affecting children, and request input from consumers regarding the quality of services they receive.

Admission Criteria

  • The program serves children between the ages of five and eighteen. Some individuals continue to qualify for the program after the age of eighteen if they are still involved in school.
  • The consumer must reside in a family type setting (including foster care).
  • The consumer must have a primary mental health, Axis I (DSM-IV) diagnosis, or co-occurring mental health/substance abuse diagnosis.
  • The client has received or is expected to receive mental health/behavioral health services for one year
  • The client must have a CAFAS/PECAFAS score of 90 or above including the parent rating scales.
  • The client must have one of the following risk factors present within the last 90 days: potential for expulsion from school, hospitalization, returning to their home community from residential or out of state placement, and/or CPS involvement.
  • The client’s legal guardian agrees to participate in all aspects of treatment.
  • The client is in need of repeated, structured, and clinically directed interventions to make behavioral changes.

Continuing Stay Criteria

The consumer remains eligible for services as long as functional impairments yield a CAFAS/PECAFAS score above 30, and the client and guardian participate in scheduled services.

Discharge Criteria

The consumer may be discharged from IHC services under the following circumstances:
  • If his/her level of functioning improves and the CAFAS/PECAFAS score is below 30. All consumers with a CAFAS/PECAFAS below 30 will be monitored for 90 days before the case is closed to assess any potential problems.
  • If the child is removed from the home and placed in a residential treatment facility or institutional setting.
  • If the child/guardian does not regularly attend scheduled appointments.
  • The consumer may also be discharged, (or transferred to an adult case management team), if he/she reaches his/her eighteenth birthday or graduates from school.

Days/Hours of Operation

The program operates Monday through Friday 8:30A.M. to 5:00 P.M.

Staffing Pattern

The Cabell In-home services team is staffed with one liaison with a BA degree in a human services field and 2 years experience, one full-time case manager with a BA degree in a human services field, and two full-time therapists with an MA in a human services field. The Mason County team is staffed by one full-time case manager with a M.A. degree in a human services field, one full-time M.A. level therapist and a full-time senior therapist. Each case manager/therapist team maintains a minimum caseload of twenty clients, and may accept up to twenty-five clients. Both teams are supervised by a .55 FTE program manager.
