Description of Program

The program is designed to provide therapeutic intervention to consumers age 18 years and older. Services are provided by Master's level, qualified staff. Interventions are grounded in an identifiable theoretical base. Services are provided in order to assist the individual in achieving an optimal level of functioning as determined by the individual and significant others. The provision of treatment and the utilization of concurrent services achieve this. These services include individual, group, family and marital therapy and case management services, as needed. This unit, at the request of consumers and community resources can provide psychological/psychiatric evaluations for all age groups.

Days/Hours of Operation

Monday - Friday, 8:00am - 5:00pm, evening appointments and groups are available also.

Admission Criteria

  • Consumer must be 18 years of age or older
  • Consumer has a behavioral health diagnosis based on DSM IV criteria.
  • Consumer demonstrates intrapsychic or interpersonal conflicts and/or desire to change behavior patterns.
  • Interventions are focused on the dynamics of the consumer's problems and are grounded in a specific theoretical base that provides a framework for assessing change.
  • The consumer's treatment plan refelects the consumer's articulated goals and documents the need for outpatient services.
  • The consumer cannot be an immediate danger to self/others that would require a more intensive level of care.

Case Management Admission Criteria

  • Consumer has less than 2 CRU and/or inpatient hospitalizations in a 12 - month period.
  • Consumer has an overall WVFA domain rating of less than 3.
  • Consumer requires two or less targeted case management hours per month in a 90 -day treatment cycle.
  • Consumer meets the criteria for targeted case management services based on qualifying DSM-IV diagnosis.
  • Transfers to more intensive case management teams can be authorized by the team supervisor to ensure the consumer's health, safety and welfare.
