Essential Outpatient Services

|
Program Description
The Essential Outpatient Program Team serves adults 18 years of age and older who have limited needs, as they primarily come to the center for medication/physician services only. Consumers served on this team meet with their Case Coordinator every 4 to 6 months and have appointments with the treating physician as appropriate. Consumers are assessed periodically to ensure this level of care remains appropriate and can be easily transferred to a higher level of care should the need arise.
Admission Criteria
- Consumer must be 4 years of age or older.
- Consumer has a behavioral health diagnosis based on DSM IV criteria.
- The consumer agrees to a low level of care, which does not include any services other than Medication Reviews.
- The consumer cannot be an immediate danger to self/others that would require a more intensive level of care.
