Essential Outpatient Services

Program Description

The Essential Outpatient Program Team serves adults 18 years of age and older who have limited needs, as they primarily come to the center for medication/physician services only. Consumers served on this team meet with their Case Coordinator every 4 to 6 months and have appointments with the treating physician as appropriate. Consumers are assessed periodically to ensure this level of care remains appropriate and can be easily transferred to a higher level of care should the need arise.

Admission Criteria

  • Consumer must be 4 years of age or older.
  • Consumer has a behavioral health diagnosis based on DSM IV criteria.
  • The consumer agrees to a low level of care, which does not include any services other than Medication Reviews.
  • The consumer cannot be an immediate danger to self/others that would require a more intensive level of care.
