Description of Program

The team serves adults 18 years of age or older who have been diagnosed with a severe, persistent mental illness. The General Case Management Team assists in a variety of ways including helping consumers access needed services and benefits. Additionally, the team provides Supportive Counseling and helps to coordinate all other adjunct services offered at the center. General Case Management services are provided with an emphasis on brokerage case management and linkage to appropriate services based on individual need.

Days/Hours of Operation

Monday Friday, 8:30 AM - 5:00 PM

Admission Criteria

  • Consumer is in need of assistance in order to properly access community services, benefits, etc. (based in part upon functional assessment data)
  • Consumer meets the criteria for targeted case management services based on qualifying DSM-IV diagnosis.
  • Consumer has an active need to improve problem solving skills.
  • Transfers to more intensive case management teams can be authorized by the team supervisor to ensure the consumer's health, safety and welfare.
