Notice of HIPAA Incident

Prestera Center for Mental Health Services has become aware of a potential data security incident that may have resulted in the inadvertent exposure of some patients’ personal and health insurance information. Although at this time there is no evidence that patients’ information was actually accessed or viewed, or any indication of actual misuse of anyone’s information, we have taken steps to notify any patients who may have been affected by this incident.
On January 19, 2018, we discovered that an unknown, unauthorized third party gained access to an employee’s email account, which prompted an investigation to determine whether the account was at risk and if sensitive information was accessed. The investigation determined that an unknown, unauthorized third-party could have viewed or accessed patients’ information, including names, dates of birth, patient account numbers, dates of services rendered, providers’ ID numbers and insurance carriers. Addresses, Social Security Numbers or financial/billing information were not involved in this incident.
Although at this time we are not aware of anyone’s information being misused, we take the privacy and security of patient information very seriously, and have taken steps to prevent a similar event from occurring in the future. Notification letters mailed on March 20, 2018, include additional information about what happened and a toll-free number that patients can call to learn more about the incident. The privacy and protection of patient information is a top priority for Prestera Center, which deeply regrets any inconvenience or concern this incident may cause.
