Primary Healthcare

Prestera Center and Valley Health Systems have joined together to improve the overall wellness and physical health status of those with serious mental illness by providing primary health care services in the community mental health setting. This four year grant was awarded by the Substance Abuse and Mental Health Services Administration (SAMHSA) on October 1, 2010.

Eligibility

Clients of Prestera Center are eligible for this program if they are a West Virginia resident, 18 years of age or older, and have a diagnosis of a serious mental illness.

Services

The Valley Health at Prestera Center team is a collaborative effort between the primary care physician, care manager (RN), psychiatrist, and therapist.

Services include integrated primary and behavioral healthcare available at a single location, referrals to peer support services, care management, and Wellness Recovery Action Plans (WRAP). Screenings measure depressive symptoms, weight, blood pressure, glucose levels, lipids, alcohol use and substance abuse. Individual and group wellness programs include smoking cessation.
